                We have used these forms for 8 years in our diesel business. Recently, Staples updated the form to include some disclosures, thus taking up the extra space at the top of the forms where we would have placed our business information sticker. Even though we don't require the extra disclosure information, we still use these forms over ordering expensive pre-printed forms.
