I had MAJOR back surgery in Jan 2010 (a lumbar fusion) and had lots of trouble sitting for longer than 15 minutes at a time in any traditional office chair. I remembered that these chairs existed, so my work offered to get one for me. So far, over the last approximately two months I've used this chair at work (during my 10 hour shifts) I've found this chair to be DARNED comfy. I am not the thinnest person in the world, weighing 160 pounds, and find only minor discomfort to the knees and to the posterior - but this is after long periods of sitting. Others in the office were intrigued and have tried it; they also find it surprisingly comfortable. The only two downfalls to this chair are: 1.) [...] 2.) Even this metal seat frame has developed a slight "creak" to it where it feels like perhaps some metal has given way very slightly, but it's never "let me down" yet!

This is my third kneeling chair and two things make me wish I had not purchased this model: The angles and height of the knee pads are not right for me . My knees hurt and my shins hurt after just 10-15 minutes. The height of the chair is adjustable somewhat, but the angle is almost like a regular chair. This causes my knees to be bent acutely and it does not encourage a straight back. I find myself hunching over no matter what height I make the seat. In addition, it is akward to get in and out of. Your feet catch on the knee pads. I almost fall every time I try to stand up. If you are really tall, this might work for you but it is worse than a regular hard kitchen chair for me. I am 5'2"

        I have arthritis in my hips and lower back and can not sit for long periods of time. Unfortunately, that is exactly what is required of me on my job. With my knee chair I can switch back and forth to have a comfortable "sit" all day.

                No this chair does not have an optional backrest.

                A backrest would defeat the purpose of the design. The chair is designed to keep your back straight. Leaning against some sort of backrest would cause you to slide forward and cause a great deal of discomfort.

                Staples has dozens of traditional chairs with backrests if you do not require a chair that is good for your back.
