I use these forms for promotional sales campaigns and also for hard copy company gift certificates when customers request them. I like the design, although I wish there was a line for expiration date under the amount. (I include that information underneath the line for amount.)

I like the fact that I instantly create a hard copy when writing the original, enabling easy tracking of recipients and amounts for each.

Ideally, there would be an aesthetically placed empty field for the company to place their logo or at least their name or slogan. That is something I would definitely request, along with the line for expiration date.

Worked in a pinch when I ran out of Gift Certificates at Christmas time. However, I prefer more of a card stock w/little more interest. It was more plain than I'd like to have used.
