2.Sweepstakes occurs monthly, beginning January 1, 2008 at 12:00:00 a.m. Eastern Time (ET) and ends December 31, 2008 at 11:59:59 p.m. Eastern Time (ET) (each calendar month is a "Sweepstakes Period").

3.Participation open to legal residents of the United States, age 21 or over. Sweepstakes is offered only in the United States and is void in Alaska, Hawaii, Louisiana, Puerto Rico and where prohibited. Employees and Agents (and their respective family members) of Staples the Office Superstore, LLC ,("Sponsor"), its parent organizations, affiliates, governors, officers, directors, agents, employees, subsidiaries and service agencies [or independent contractors] are ineligible to participate or win a prize. A PURCHASE DOES NOT INCREASE ONE'S CHANCE OF WINNING.

4.To Enter: There are two ways to enter. (1) Customers who receive an official survey code can complete a customer satisfaction survey online or by phone during a "month" of the Sweepstakes Period (a "month" being defined as between 12:00:00 a.m. (ET) on the first day of a given calendar month and 11:59:59 p.m. (ET) on the last day of that calendar month) in order to enter that month's drawing. To complete survey online, log onto: http://www.staples–survey.com. To complete survey by phone, call toll–free 1–800–890–7305. You will be automatically entered upon proper completion of the survey (note: survey participants have 7 days from the date they receive a survey code to complete the survey; survey code is non–transferable and not redeemable for cash or credit); (2) Alternate Entry: No Purchase Necessary. Legibly hand print on a postcard, your complete name, street address (no P.O. Boxes please), including city, state and zip code, daytime phone number (including area code) age and e–mail address (if available) and mail to: 2008 Staples Customer Satisfaction Survey Sweepstakes, P.O. Box 779, Georgetown, CT 06829–0779. All mail–in entries must be postmarked on or before the last day of a month and received by the 7th day of the immediately following month for entry into that month's drawing. If the last day of a month and/or the 7th day of the immediately following month falls on a Sunday or a government Holiday, the entry may be postmarked or received (as appropriate) on the immediately following business day. Each postcard entry must be mailed separately. No mechanical reproductions or facsimiles of postcard entries will be accepted. Limit only one entry per person or household each month, regardless of method of entry. Entries received for a given month's drawing will not be retained for subsequent months' drawings. Only complete entry forms are eligible. In case of dispute as to identity of an online entrant, entry will be declared made by the authorized account holder of the E–mail address used to enter the Sweepstakes at time of entry. "Authorized Account Holder" is defined as the natural person who is assigned an E–mail address by an Internet access provider, online service provider, or other organization (e.g., business, educational, institution, etc.) responsible for assigning E–mail addresses for the domain associated with the submitted E–mail address. Any other attempted form of entry is prohibited; automatic, programmed, robotic or similar means of entry are not permitted. Sponsor and its promotion and advertising agencies are not responsible for lost, late, damaged, incomplete, illegible, misdirected or postage due entries/mail or for technical errors of any kind including computer/information network failures, hardware, software, telephone or other communications malfunctions of any nature, errors or failures of any kind, lost or unavailable telephone or network connections, Website, Internet, or ISP availability, unauthorized human intervention, traffic congestion, incomplete or inaccurate capture of entry information (regardless of cause) or failed, incomplete, garbled, jumbled or delayed computer transmissions which may limit a participant's ability to take part in the Sweepstakes. All entries become the sole property of the Sponsor and will not be acknowledged or returned. CAUTION: Any attempt by an entrant to deliberately damage any website or undermine the legitimate operations of the Sweepstakes is a violation of the criminal and civil laws. Should such an attempt be made, the Sponsor reserves the right to seek damages from any such entrant to the fullest extent permitted by law and disqualify such entrant from the Sweepstakes. Sponsor's computer shall serve as the official clock for determining timeliness of receipt of survey entries.

 
5.Drawing: Winners will be selected in a series of twelve (12) monthly random drawings with one drawing (and one winner) per month; that is, one drawing per month from January 2008 to December 2008. Each month's drawing will consist of all eligible entries received for that month (see Section 4 "To Enter", above). All drawings will be conducted by PHD Marketing Services LLC, an independent judging organization, on or about the fifteenth day of the immediately following month. Odds of winning depend on the total number of eligible entries received for each month's drawing. Judges' decisions will be final and binding in all matters relating to the Sweepstakes. Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify, or suspend this Sweepstakes should (in its sole discretion) causes beyond its control corrupt or affect the administration, security, fairness or proper conduct of the Sweepstakes. In such case, Sponsor will select the winner(s) for effected drawing(s) from all eligible survey entries received for a given effected month prior to and/or after (if appropriate) the action taken by Sponsor and all eligible mail–in entries received for a given effected month or by some other means determined by Sponsor in its sole discretion as being fair, appropriate and consistent with these official rules. By entering Sweepstakes, entrants agree to be bound by these official rules and the decisions of the judges.

 
6.Prize: One (1) Grand Prize for each month's drawing: $5,000 Staples Shopping Spree. A Staples store gift card with a credit balance of $5,000 will be established in the name of the winner and is redeemable at any Staples store in the United States. The Staples Gift Card is NOT redeemable, transferable or exchangeable for cash and cannot be applied to existing credit balances. If winner uses Staples store gift card to purchase goods costing in excess of $5,000, winner shall be solely responsible for such amount in excess of $5,000. A Staples store gift card is also subject to terms and conditions of use, as indicated thereon. The prize awarded in connection with the Sweepstakes is nontransferable and non–endorsable. The prize (or any portion thereof) cannot be sold or transferred. No prize substitutions allowed, except at Sponsor's sole discretion in the event of prize unavailability for any reason, and then only for a prize of equal or greater value. All federal, state and local laws and regulations apply. Limit one prize per person/household/family/address.

 
7.Notification: Each prize winner will be notified by phone, e–mail [if provided] and overnight mail. Winner will be required to sign and return an Affidavit of Eligibility and a Liability and Publicity Release (where legal) within 10 days of notification. If winner cannot be contacted, if prize is returned as undeliverable, or in the event of noncompliance with these Official Rules, the prize will be forfeited and an alternate winner will be selected. Upon forfeiture, no compensation will be given.

 
8.Conditions: All taxes are the sole responsibility of the winner accepting the prize. A 1099MISC Federal tax form will be issued to the winner for the value of the prize. Participation in Sweepstakes and acceptance of prize constitutes winner's permission to use his/her name, address (city and state only), likeness and any statements made by winner regarding the Sweepstakes or Sponsor for advertising, trade and promotional purposes without additional compensation, except where prohibited by law. By acceptance and use of prize, winner agrees to hold harmless and indemnify Sponsors and their respective parent organization, affiliates, governors, officers, directors, agents, employees, subsidiaries and service agencies [or independent contractors] against any and all liability, damages or causes of action (however named or described), with respect to or arising out of entrant's participation in the Sweepstakes, or the receipt or use/misuse of the prizes awarded herein. These parties, their respective affiliates and subsidiaries, advertising/promotional agencies, and each of their directors, officers, employees, agents, successors and assigns have no liability or responsibility for any personal injuries, losses or damages or claims of any kind resulting from participation in the Sweepstakes, acceptance, possession or use/misuse of any prize. Entrants who do not comply with the Official Rules, or attempt to interfere with this Sweepstakes in any way shall be disqualified. There is no purchase or sales presentation necessary to participate.

 
9.Additional Terms: Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds, in its sole discretion, to be tampering with the entry process or the operation of the Sweepstakes or Web site. For details regarding collection of information from users of the Web site (including entrants), please consult the privacy policy on the Web site. Additionally, Sponsor reserves the right to prosecute any fraudulent activities to the full extent of the law.

 
10.WINNERS LIST: For the name of the monthly prize winner, send a self–addressed stamped envelope, postmarked no later than the 15th of the month immediately following to: 2008 Staples Customer Satisfaction Survey Sweepstakes [specify month] Winners List, P.O. Box 746 Georgetown, CT 06829–0746. Residents of VT only may omit return postage.
