These were ordered for the Supervisor of Maintenance. He had specifically requested this product and is well pleased with the forms.

Dropped off delivery
Good morning
I do have an issue with this Delivery……I am Jim Hoffman (Purchasing Manager) and I didn’t sign for this order as I am not in the receiving department
This has happened a couple times as they just leave or drop off the order/delivery
Who do we need to speak to, to fix this problem

Our tech gives our customers the top copy, I keep yellow copy in customers file and the hard copy is for our accountant. Works great for us.
