Description Incorrect -- Description shows 50 sheets per pack. There were 100 per pack which was fine. However the description shows each pack includes 1 W-3 and 1 1096 transmittal. No W-3 or 1096 were included.

Clean Effective but thin
Forms are well priced clean and effective. It is worth noting that the paper weight is less than expected. I'm not sure what the weight is but it is noticeably thinner than previous forms I still had left over from another provider.

I ordered 2 packets because it stated that there were 100 forms per packet however when I went to use them there were only 50 forms per packet and since I need them ASAP I had to just copy the form on my copier and therefore the remaining forms did not have perforations and employees will have to cut them. Otherwise the forms worked perfect!
