Description shows 50 sheets per pack. There were 100 per pack which was fine. However, the description shows each pack includes 1 W-3 and 1 1096 transmittal. No W-3 or 1096 were included.

Clean, Effective, but thin
Forms are well priced, clean, and effective. It is worth noting that the paper weight is less than expected. I'm not sure what the weight is, but it is noticeably thinner than previous forms I still had left over from another provider.
