Contact Customer Service if you are having issues getting the correct form. I was relieved that support gave me the right answer and was helpful. I respected her so much more when she asked me to hold so she could get additional information.

These are great and perfect for printing from Quickbooks. Great value as well! Unfortunately at this present time the description on Staples site and/or from the manufacturer does NOT tell you that the employee filing instructions are printed on the back. Just what we need!
They have small government type font on the back (same as a typical W-2 form) that was not expected but they look great and are just what we need to print W-2s from Quickbooks.

Perforated Blank W-2 Forms 3-up Just right
These W-2 forms worked perfectly with my QuickBooks Premier Accountant's Edition software as I printed and mailed over 100 forms and the envelopes matched perfectly. Form folding to envelope was excellent. No Adjutsments

Tax Time Ease
If printing employee tax docs typically many purchase preprinted with year and all fields. Using this paper saves money as all fields are printed at time of doc completion.
