Accuform Signs® "TO FIRE.." PF-Cardstock fire inspection tag has record printed at the back with a blank grid provided for writing data. Tag measuring 5 3/4" x 3 1/4" features 10 mil rigid surface and is water resistant, sold as 25 per pack.
