questionable quality
while I am very happy to order the HCFA 1500 (02-12) form through Staples the last batch I received was better quality. The current supplier is using thinner paper and the ink is lighter. My printer is not feeding these as accurately and as a result some are coming out with mis-aligned printing. Thus I must re-print and use extra forms. While I'm sure Staples doesn't h...

Laser cut insurance forms
I am certainly glad I could pick up the phone and order these forms. My office staff said they were running low and I simply called staples and ordered some paper that was on sale by the way and these forms. This saved me a trip across town and I did NOT have to lift and load and unload all the heavy boxes of paper. We use a lot of paper in our "paperless" medical offic...

I had such a hard time ordering the new 02/12 version of the Health Insurance forms for my business. The other online stores require that you purchase 1 000 or more forms! Quill allowed me to buy exactly what I needed and were delivered the very next day.

Came early which was very helpful!
I don't accept much insurance so when the auto insurance company said they can't accept electronic submissions I needed new forms fast. They came in just a few days not the two weeks I was quoted. No complaints.
