Pretty Standard
I got this for the Transfer Station at work. Although we don't use them often they were great since they are three part forms. Pretty inexpensive too.

it has all the information i need
it has all the information I need for bill of lading. it is clear easy and gets my job done in the most convenient way. just wish that the price is a little high.
