Simple Form
Very generic, non-specific form. Just what I needed for the many scenarios we'd use it for. Plenty of lines and room for notes. Carbonless works great too (nice and dark copy).

We have been buying the Purchase Order Book, Gold Standard by Rediform for many years. This Adams 2-Part is a very poor substitute.

Adam 2-part carbon less purchase request

Excellent For Purchases
I had a hard time finding carbonless purchase orders and, in the past, had to use a sales book to write down what I needed to purchase with carbonless copies. I began to think that this type of book had been discontinued since a lot of people probably just use their computers or a simple list, but I like the convenience of this book.
