I use this low tech approach to make an invoice and/or receipt at the time of service. It gives me a copy of the transaction for my records.

Easy to read right down to the third copy!
It's an orders book but converts easily to an invoice book for me. It provides 2 copies for the client and one for me. That way the client has one for their records and mails the second copy with payment.
