14 3/8x12x3 1/2 Partners Brand Corrugated Boxes 25/Bundle (14123).
A simple item. Hard to go wrong unless you choose the wrong size. The variety of sizes at the Staples website is much greater than at Amazon. They cost 98 cents each were well packaged (banded and shrink wrapped) and arrived in a few days.

Length is 12.5" not 12"
The title says 12" but it's actually 12.5" as noted in the specifications. That messed me up because FedEx and UPS will charge it at 13" and I get hit with a 6 lb "dimensional weight" charge (minimum weight charged based on box size). Makes a big difference in price for my international shipments. I've bought slightly smaller size of this box from staples repeatedly an...

Ok for the limited 3" size options
Ordered these to replace the 11.75" x 3" x 15.25" size that is no longer offered. These are not as heavy of cardboard and the different opening style lacks the overlapping ends that gave the other style better strength when folded up. I won't order these again and may have to move to U-Line for a 3" width box more suitable for my needs.
