Gift certificate booklet
Great for the price. Easy to keep track with the stubs you leave behind. I purchased some gold envelopes to make it more fancy.

Fundraising Supplies: Gift Certificate Forms
My printer died and I needed a gift certificate for one of the baskets I was making up for a fundraiser. Thought of Staples and went with an 8-1/2 x 11 paper pre-printed gift certificate in mind.
