Available in the small qty I needed, as I rarely submit claims on paper. I tried 1-hr in-store pickup and it probably saved me some time.
These forms are basically all the same. Since I resent having to pay for the standardized printed forms in order to submit a government claim, I wish to pay as little as possible.

cms1500 health insurance claim forms.
I wasn't sure if these would worl with my printer. They said they were for a laser printer and I don't have one. Will they print on a nonlaser printer? Thanks, Virginia

I didn't see where there was a template for these - found one online and had to play with the print settings to get it to align right -this should be easier
