The description on this product was very misleading. In the description it lists getting 50 forms each of copies A and B and 100 forms of copy C, D, 1 and 2. You only get 25 forms each of copy A, B, C and D and copy C and D have been combined with copy 1 and 2. This is false advertising. It says 50 of each form but there are only 25 of A & B!! Very disappointed. Now I require more forms!

Incorrect Product Quantity
These are nice forms, but the product description is incorrect. There are only 50 envelopes, not 100 and do not miscalculate forms for sheets. This pack will only cover 50 employees.

Not this year!!
Have used these forms in the past with no issues, but this year it was impossible to align them in QB. Get one aligned correctly and the other one was way off. Not Happy!!! Probably will not use TOPS again!

Exposes Employee's Social Security Number
The forms work fine with QB. BUT the envelopes are cut so that the employee's social security number is clearly and easily seen by everyone who may handle the envelope.
