I was told by a Staples Employee that this was what I should buy when I asked about a Limited Power of Attorney for medical purposes but that is NOT any part of what’s in this packet or what you get when you download it as they instruct. If you need a Medical Power of Attorney that’s a completely different packet. However the Staples store manager allowed me to return ...

An inexpensive legal document
The form was easy to use as a draft. The final form is found on the internet easy to complete and ready to print sign and notarized. If I had known its location on the internet I would not have needed to buy the package.
