Medical Arts Press® 11 Pt. Manila End-Tab Pocket Folders
Really good fro either medical or dental offices. It's easier to flip through patient information because its like a book.

patient charts
I like the chart because it has the pocket and fastener attached, but i do not like the fact that the tab is on the wrong side. When you stand the chart upright, the name tab is on the bottom and get smashed. Also i wish the pocket was closed. Papers fall out on the open end.
